It’s easy to become a patient at Acorn Pediatrics! If your child has been a patient at another practice, simply fill out our transfer of medical records form and send it to your previous practice, then fill out a patient packet and send it to us. Now you're ready to make your first appointment!
Don’t forget to ask your previous practice if there is a fee for transferring or obtaining a copy of your medical records. When there is a fee, it must be considered reasonable, and your previous practice cannot withhold or delay providing your medical records solely on the grounds of nonpayment.
