Special Needs

We believe in being a medical home for all kids. Children with complex illnesses and special needs are always welcome here. We offer chronic illness management, close coordination with subspecialists and other providers, and we respect your extensive knowledge of your child and their needs. Our office exceeds ADA requirements and includes accessible exam tables, wheelchair scales and a push-button entrance.

Call us if you are interested in being connected with someone who's been there or if you are interested in helping other parents, please let us know!

If you are completing these forms for someone else, such as your child, answer all the questions on behalf of your child, not yourself. For example, only list the income and assets of the child seeking eligibility. Bring your completed Medicaid Application and Appendix D to your local Department of Social Services (DSS) and request a screening for Long Term Care.
